Fatigue isn't a diagnosis. It's a symptom, & symptoms have causes. Most of what drives low energy is genuinely influenceable – the nutrients, the stress physiology, the blood sugar, the gut, the sleep quality, the load your body is carrying from things like mould or heavy metals, & how well your cells turn fuel into energy. That's a lot of levers, & most of them are within reach.

Have you got the raw materials?
Iron, B12 & folate, thyroid hormones, blood sugar, vitamin D. These are the building blocks of daily energy, & if one of them is short you'll feel it long before anyone calls it a deficiency.
This is where the single most commonly missed finding in tired clients lives – & it is usually sitting inside a result someone has already told you was normal. "In range" & "enough for you" are not the same thing.
Your bloods answer this question well. It's exactly what they're built for, & it's why we start here.
What's draining the tank?
This is where most investigations stop. You can have perfectly adequate raw materials & still be exhausted, because something is using them up faster than you can replace them.
Chronic stress reshapes your cortisol rhythm, which is why you can be shattered in the morning & wired at 10pm. Inflammation costs energy. An unhappy gut costs energy twice over – once running the inflammation, & again by not absorbing the nutrients from question one. Poor sleep quality, as opposed to poor sleep quantity, costs more than almost anything.
Your bloods hint at some of this & miss most of it. A blood test can't tell us what your stress physiology is doing across the day, how well you're absorbing, or what your sleep is actually like.
Are your cells making energy from it?
This is the part nobody measures. Every cell in your body has mitochondria – your cellular batteries – & their job is to turn fuel & oxygen into usable energy. They need specific nutrients to do it, & they're vulnerable to inflammation, toxins & oxidative stress.
You can have the raw materials, no obvious leaks, & still feel flat, because the conversion itself isn't happening properly.
Your bloods can't show this at all. It needs different testing altogether, & more often than not it needs the full story of how you got here.

Fatigue is Rarely One Thing
Here's what I see over & over in clinic: fatigue is almost never a single cause. It's usually two or three interacting, which is precisely why single-marker testing misses it.
Nutrient Status
Iron, ferritin, B12, folate & vitamin D sit behind every bit of energy you make. They're easily depleted by stress, poor absorption, some medications & your own genetics – & having enough circulating isn't the same as getting it inside your cells.
Thyroid Function
Standard testing usually stops at TSH, which tells us a message was sent & very little else. Sluggish conversion into the active hormone, or early autoimmune activity, can drain you for years while your results look unremarkable.
Stress, Trauma & Cortisol
Long-term pressure reshapes your daily cortisol rhythm: tired but wired, shattered at 7am & unhelpfully alert at 10pm. Old trauma counts here too – a nervous system that learned to stay on guard often stays that way long after the circumstances have changed. Neither resolves by pushing harder.
Blood Sugar & Insulin
Insulin resistance can build quietly for years, & fluctuating energy may be one of the clues – afternoon crashes, waking unrefreshed or reaching for something sweet at 3pm. Often visible in results long before anyone mentions pre-diabetes.
Gut Health & Absorption
You're not just what you eat, you're what you absorb. Gut inflammation costs energy twice over – once running the inflammation itself, & again by undermining the nutrients your cells are waiting on.
Sleep Quality

Then we watch what changes. Your symptoms get tracked month by month & I'll tell you what's shifting, including the things you'd stopped noticing. Follow-ups keep it moving, & repeat testing (bloods or other) confirm it when the timing's right.

Why am I always tired when my blood tests are normal?
Standard reference ranges are designed to detect disease, not to define good health. You can sit "in range" & still be a long way from the optimal zone where energy & clarity live. Reading your results against narrower functional ranges, & as a connected pattern rather than isolated numbers, very often reveals the explanation that's been there all along. & where it doesn't, it tells us where to look next.
Can a naturopath help with chronic fatigue?
Yes. Fatigue is a symptom of underlying dysfunction, & naturopathy is well suited to investigating it, because your body gets looked at as one connected system. Iron, thyroid, cortisol, nutrients, blood sugar, gut health & sleep all interact, & the answer is usually in how they combine rather than in any single marker.
What's the difference between chronic fatigue & ME?
"Chronic fatigue" is often used loosely to mean being tired for a long time, which could have any number of causes. ME/CFS – myalgic encephalomyelitis, sometimes called chronic fatigue syndrome – is a specific diagnosed condition with its own criteria, the central one being post-exertional malaise: a disproportionate crash in the hours or days after activity that would once have been easy.
The distinction matters practically, not just technically. With ME/CFS, pushing through & adding exercise can make things considerably worse, which is the opposite of the advice that helps most tired people. If that pattern sounds like you, say so early – the approach changes, pacing comes first, & diagnosis belongs with your doctor.
Can you help with long covid or post-viral fatigue?
Energy that never came back after a virus is a real & well-recognised pattern. A great deal of what sits underneath post-viral fatigue, such as Long Covid, is the sort of thing this work addresses: mitochondrial function, nutrient status, inflammation, gut health & a stress response that hasn't switched off. Progress is usually slower than people want & pacing matters enormously, so expect an honest conversation about timeframes rather than a quick fix.
Could my fatigue be autoimmune?
It can be. Fatigue is one of the earliest & most common symptoms across autoimmune conditions, & it often turns up well before anything is diagnosed. Diagnosis sits firmly with your doctor, & if your results or symptoms point that way you'll be told clearly so you can follow it up. What I look at is everything influencing the picture underneath – inflammation, gut health, nutrient status & stress physiology – which is work worth doing whether or not a diagnosis eventually arrives.
Can you help with fibromyalgia?
Fibromyalgia is diagnosed by your doctor, & this work sits alongside their care rather than replacing it. What's often useful is looking at the things that commonly travel with it: sleep quality, nutrient status, gut health, inflammation & stress physiology. Many people find that addressing those makes a meaningful difference to how they feel day to day.
What tests should I have for fatigue?
It starts with comprehensive standard bloods – a full thyroid panel, iron studies & ferritin, B12 & folate, blood sugar & inflammation markers – read through an optimal health lens. Where the picture is still incomplete, targeted functional testing may add real value: cortisol patterns across the day, gut analysis, or cellular energy markers. Only ever what your situation genuinely calls for.
Why isn't a blood test enough on its own?
Because bloods are a snapshot of what's circulating, & a lot of what drives fatigue isn't circulating. They're excellent at showing whether you have the raw materials for energy. They're much weaker at showing what's draining you, & they can't show whether your cells are converting fuel into energy properly. That's why the bloods are the starting point rather than the whole investigation.
How long until I feel different?
Honesty matters more than hype here. Simple nutrient issues can shift within weeks. Longer-standing hormonal, gut or cortisol patterns typically take a few months of consistent, targeted support. You'll always get a realistic picture upfront, & repeat testing to show progress objectively.
Can you help if I don't live on the Kapiti Coast?
Absolutely. Around half of all clients work with Caro entirely online, from all over New Zealand & Australia. The process is identical, & any test kits are couriered straight to your door.
Will you work alongside my GP?
Yes, always. Medication is never stopped or changed except by your prescriber, & with your permission, collaborative-care letters keep your GP in the loop.
Could my fatigue be something serious?
Occasionally, persistent fatigue signals something that needs medical attention – which is another good reason to start with proper testing. This work happens alongside your GP, never instead of them, & anything that warrants medical follow-up gets flagged for exactly that.
Some things need a doctor sooner rather than later. If your fatigue comes with new shoulder or hip pain & significant morning stiffness, unexplained weight loss, drenching night sweats, or new severe headaches or changes to your vision, please see your GP promptly rather than booking with me first.
