When it's not firing on all cylinders you can feel absolutely rubbish - exhausted, foggy, cold when nobody else is, hair thinning, weight that won't budge - even when your doctor says your tests look fine.
Here's the thing most people are never told.
Standard testing usually measures TSH, & sometimes T4. TSH is your brain's message to your thyroid – essentially "Hey thyroid, wake up!" Then it’s telling your thyroid to “Make more (or less) hormones”. TSH tells us the message was sent. It tells us nothing about whether your body converted your inactive hormone into the active form needed to do the work, or whether the active hormone is actually getting inside your cells to do its job.
It's like confirming your phone has a full battery, but never checking whether it can connect to the network – or whether your apps actually run.
So if you've been told everything's normal & you still feel dreadful, you're not imagining it. You've just had 1 part of a 3-part picture measured.
The first job, then, is to get the fuller set of numbers & read them properly. Most people have never had that done. And then comes the part almost nobody mentions. Even with a complete set of results in front of us, there is far more to how your thyroid actually functions than any blood test can show.
Your bloods tell us what is happening. They can't tell us why – & with thyroid, the why is very often somewhere else in your body altogether.
That's not a reason to skip the bloods. It's the reason to start there, & then keep going.

Your thyroid produces mostly T4 – the storage form – plus a little T3, the active one. This is the part standard testing looks at, & it's a genuinely useful starting point.

T4 has to be converted into T3 before it can do anything useful. Under stress, inflammation or nutrient shortfalls, your body can make reverse T3 instead – the evil twin – which tells your system to slow everything down & conserve energy.

Is it getting into your cells?

Your Thyroid Rarely Struggles Alone
Here's what I see over & over in clinic:
The thyroid is usually the first system to complain when something else is struggling. It's the canary in the coal mine – or the check engine light, for the younger crowd. Two relationships matter more than any others:
Your gut.
A meaningful share of your T4-to-T3 conversion happens in your gut. Gut inflammation directly affects thyroid function, & the bacteria living there help regulate your thyroid hormones.
Your adrenals.
Chronic stress makes your body prioritise cortisol over thyroid function. High cortisol blocks conversion & pushes production toward reverse T3. Stressed adrenals & struggling thyroids turn up together so often they're practically a package deal.
And in perimenopause the whole thing gets more interesting again. When oestrogen starts waving goodbye, your thyroid often throws a bit of a wobbly too – these hormones are practically besties.
This is why treating the thyroid alone so often disappoints. Supporting a struggling thyroid while ignoring stressed adrenals or an inflamed gut is like bailing out the boat without ever finding the leak. You can do it for years, & you'll be tired for all of them.
Which is also why your bloods are the beginning of this conversation, not the end of it.
If You’re Already on Thyroid Medication
If you're on levothyroxine or similar & still feel like you've been hit by a bus, you are absolutely not alone.
Your medication supplies T4. Your body still has to convert it, transport it, & get it inside your cells – & every one of those steps can be blocked by the things above. You can be perfectly medicated & still unwell.
To be completely clear: your medication is prescribed & adjusted by your doctor, & that's exactly where it belongs. Nothing here involves stopping or changing it.

But numbers on their own don't mean much. The same result can mean 2 completely different things in 2 different people. Step one tells us what is happening, & for a good number of people that's genuinely where the penny drops. But it almost always raises a sharper question: why is this happening to you?
Why isn't the conversion working? What's driving the inflammation? Why is your body behaving as though it needs to conserve energy?

Then we watch what actually changes. Your symptoms get tracked month by month – energy, temperature, sleep, mood, hair, digestion, how your brain is behaving – & I'll tell you what's shifting. That matters more than you'd think, because improvement is gradual & people genuinely forget how bad things were. One of my favourite parts of this work is pointing out something that's quietly resolved while you weren't looking.

Hashimoto’s & Grave’s - Autoimmune Thyroid
If your antibodies are raised, the conversation changes. The thyroid isn't really the problem – it's the target. The question becomes why your immune system is behaving this way.
Hashimoto's is the common one. In countries like New Zealand & Australia it's behind most underactive thyroids. The immune system gradually slows the thyroid down, & the picture is the familiar one: exhaustion that sleep doesn't fix, weight that won't budge, feeling cold when nobody else is, dry skin, thinning hair, constipation, brain fog & low mood. Many people have it for years before anyone diagnoses it, because TSH can stay respectable for a long while as antibodies quietly climb.
Graves' works the other way. The immune system drives the thyroid too hard, & the result is the opposite picture: racing heart, weight dropping off you, heat intolerance, shakiness, anxiety, trouble sleeping, sometimes changes to the eyes. It needs medical management, & that is not negotiable.
What both have in common is that the immune system is the thing worth understanding. That's where investigation earns its keep: gut integrity, inflammatory drivers, nutrient status, stress physiology, & sometimes environmental factors. There's also a well-recognised relationship between gluten & autoimmune thyroid conditions, worth exploring properly rather than guessing at.
None of this replaces medical management, & with Graves' in particular your specialist leads. My work sits alongside theirs, on the terrain the medication doesn't reach.

What is Hashimoto's thyroiditis?
Hashimoto's is an autoimmune condition where the immune system gradually attacks the thyroid, slowing it down over time. It's the most common cause of an underactive thyroid. It's diagnosed by testing thyroid antibodies (TPO & thyroglobulin) alongside thyroid hormones – & because antibodies can be raised for years before TSH shifts, plenty of people feel unwell long before anyone diagnoses it. Diagnosis & any medication are your doctor's domain. What I look at is everything influencing the immune picture underneath: gut health, inflammation, nutrient status & stress physiology.
What is Graves' disease?
Graves' is the other side of the autoimmune coin. Instead of slowing the thyroid down, the immune system drives it too hard, producing an overactive thyroid. Typical signs include a racing heart, unexplained weight loss, feeling hot when others don't, shakiness, anxiety & poor sleep, & sometimes changes to the eyes. Graves' needs medical management & your specialist leads – please don't delay that. Alongside their care, there's useful work to be done on the things that influence immune activity & on replenishing what an overactive thyroid burns through.
My TSH is normal – can I still have a thyroid problem?
Yes, & it's more common than you'd think. TSH is your brain's message to your thyroid, not a measure of what's reaching your cells. You can have a perfectly respectable TSH while your active T3 sits at the bottom of the range, or while thyroid antibodies are quietly climbing. This is exactly why looking at the fuller picture matters.
What is a full thyroid panel?
TSH, Free T4, Free T3, reverse T3 & thyroid antibodies (TPO & thyroglobulin) – ideally alongside full iron studies, zinc, vitamin D & B12, since thyroid hormone production & conversion depend on them. Not every marker is routinely funded, & we'll talk through what's realistic for your situation.
Will my GP order all of these?
Sometimes yes, sometimes not – funding rules mean some markers aren't routinely covered. The pathology request form includes what's most useful, & an honest conversation about wanting to understand your own health usually goes down well. Where markers aren't available through your GP, self-funded options exist & we can discuss them. It also helps to advise your GP if you have any family history of autoimmune disease.
Can a naturopath help with hypothyroidism or Hashimoto's?
Naturopathy doesn't replace medical diagnosis or management of thyroid disease – & it shouldn't. What it can do is investigate & support everything around it: conversion, nutrient status, gut health, inflammation & stress physiology. For a lot of people, that's where the real work sits.
I'm on levothyroxine & still feel terrible. Why?
One of the most common reasons people get in touch. Medication supplies T4 - your body still has to convert it to active T3, transport it, & get it into your cells – & that depends on nutrients, gut function, liver health & cortisol patterns. If any of those are compromised, you can be well medicated & still unwell. Worth investigating, with your prescriber staying in charge of the medication itself.
Will you tell me to stop my thyroid medication?
Never. Medication is prescribed & adjusted by your doctor, & that stays exactly as it is.
Does perimenopause affect my thyroid?
Often, yes. As oestrogen starts declining, the thyroid frequently throws a bit of a wobbly too – these hormones are closely connected. Your thyroid also becomes more sensitive to stress, nutrient gaps & any lurking autoimmunity during this transition. Looking at both together tends to work far better than tackling either alone.
Can you help if I don't live on the Kapiti Coast?
Absolutely. Around half of all clients work with me entirely online, from all over New Zealand & Australia. Test kits, if functional testing is of interest, are couriered to your door.
